ENGLEWOOD, Fla. - The state DMV is investigating how an Englewood woman was mailed a new driver's license with a crude phrase listed as her address.

According to NBC 2 in Fort Myers, the woman got the license in the mail with a letter from the Department of Highway Safety and Motor Vehicles asking her to verify that the information is correct and to destroy the old license.

The DMV was unable to explain how it happened, but a statement issued by a spokesperson explains that: "We have opened an investigation into this matter to determine who and how this misinformation was provided...we have already reissued the customer's drivers license at no cost.
